Policy on Healthcare Benefits (effective January 1, 2016)


ENROLLMENT FOR EMPLOYEE HEALTH INSURANCE

Enrollment for newly hired employees

Employees newly hired have a 30-day open enrollment period in which to enroll in health coverage. Contact awagner@iowaepiscopal.org for enrollment information.

Annual Enrollment for all eligible employees in the Diocese of Iowa

  • 2024 Annual Enrollment runs through November 15, 2023.

  • New Dental Insurance Vendor or 2024! Dental insurance will be changing to Delta Dental beginning January 1, 2024. Delta Dental has the largest network of dentists nationwide.

  • The 2024 annual enrollment is an ACTIVE ENROLLMENT. This means that all employees currently enrolled in a health and dental plan MUST sign into the online site, MyCPG Accounts to select a new dental plan for 2024. Instructions to login were sent in a mint green envelope to all current employees enrolled in a health plan.

  • During annual enrollment:

    • Current plan members MUST select a dental plan for 2024, and may change their health plan selections for the following year.

    • Eligible employees not currently enrolled in an Episcopal health or dental plan have the option to join a plan. Complete the Enrollment Form and return to Anne Wagner BEFORE November 15.

    • Eligible non-participating dependents may be added to a member’s plan, and participating dependents may be removed from a member’s plan, without the need to demonstrate a qualifying event.

HEALTH PLANS & COSTS: 

In 2023 and 2024, the approved plans will be based on 2-tiers of either Single or Family coverage. There are three plan options for active employees; one PPO and two high deductible plans.

For active employees 65 and enrolled in Medicare Part A there is a Medicare Secondary Payer plan (MSP PPO 80). If you are approved and enrolled, Medicare is the primary payer of your claims covered under Medicare Part A. The Anthem PPO 80 MSP/SEE Plan will act as secondary payer of claims. The Plan will coordinate benefit payments with Medicare so that any claims not paid by Medicare will be processed under the MSP/SEE Plan. If you are enrolled in Medicare Part B, the Plan will coordinate with Medicare. Otherwise, for all Benefits covered by Medicare Part B, such as doctor visits, outpatient procedures, and some prescription drugs, your Anthem plan will remain the primary payer of your Benefits.

ELIGIBILITY

Employees:

  • Employees that are normally scheduled to work 1,000 or more compensated hours per year in the Diocese of Iowa are elligible to participate in diocesan-sponsored health plans.

  • Employees that work 1,500 or more hours per year (~30 hours/week) must be enrolled in an approved Episcopal Health Plan unless they have coverage from an approved source. Approved sources may include coverage through a spouse or partner's employment, benefits through a government-sponsored program such as Medicaid or TRICARE, or coverage from a previous employer.

Dependents:

  • A Spouse

  • A Child who is 30 years of age or younger on December 31st of the current year

  • A Disabled Child, 30 years of age or older on December 31st of the current year, provided
    the disability began before the age of 25
